Abstract
Prescribing involves careful considerations of outcome, duration of treatment and follow-up, and such considerations should be communicated to everyone involved in the medication process. Hospital admission and discharge involve a high risk of medication errors caused by unclear division of responsibilities, lack of communication, frequent exchanges between synonymous and analogous drugs, polypharmacy, compliance issues and insufficient education and training of staff. There is an obvious need for a joint internet-based online prescription system covering all physicians.
